80mg Methadone to Bup, what to do??

Kind of in a jam and have to go back onto suboxone.
I'm currently on an 80MG daily dose of methadone.

Has anyone switched over from 80 or more??
I remember it was advised not to switch from anything more than 30...

Should I just stop the meth and go back on dope for a few days,
then wait til i get sick and start the subs??

IDK what to do....
Any advice and or experience with switching to bup from an 80ish dose of meth would be appreciated!!

I think an 8mg sub would probably keep you from being sick, but since you're used to being fully opiated, it might be kind of uncomfortable still. You should try and taper down a little bit, but you wouldn't be puking and completely unable to sleep I don't think.

I would switch to a short acting opy first then try the subs, it seems to go alittle smoother for most people but you can also jump from the 'done to subs after 3-4 days.

In Australia a switch from methadone to bupe is only allowed when dosing is below 20mg of done..I guess there is some good reasoning behind this..?

Hi. i just switched to subs lately. i was at 60mg of methadone (plus short acting chipping) and brought it down to 30mg in two weeks, so i took about 20mg dilaudid intranasal a day to compensate and "saturday i took 15mg meth, sunday 0mg, monday subs" (8mg to start, ended 5 days later at 26mg sublingual - yes it must be too high but).
I'd say your meth dose is quite high, i think could compare to ... maybe 15-25mg subs(??-probably less it seems), yet you would have to wait a long time to pick up subs, 3-4 days at 0mg(...) to start if you dont cut down. definitely try to get on short actings for a few days and then one day with nothing (at all) and you should be good.

Where I live it's 15 mgs (and it still sucked..).

Make sure you're off methadone for at least 3 days! You don't want precipitated withdrawals..

If it were "me"... i would taper down on the methadone to about 20-30mg. Then switch over to a short acting opie for about 4 days. Then let yourself get TOTALLY sick...then dose a very small amount of subs at a time.

agreed, i have always heard one must get down to at least 40 mg of done to make the switch, or it wont be pretty.

i think your comment about tapering then switching to something short acting is the best idea, that will take care of the 4-5 days one would have to wait for all the done to clear out and you only have to wait about 12-16 hours to switch from a short acting opiate to sub. some say wait at least 16, but i have a friend that has done the 12 or 13 hour switch with no problems. as soon as he felt WD, he was fine with switching to sub. i guess everyone is different though, and sub is an odd drug, so what may work fine for one, may throw another into precipitated WDs.

i would not want to wait the 4 days or so going straight from done, while i have never taken meth, i cant imagine going 4 days in WD (would seem like a month) even if the WD didn't set in for 2 days, just knowing it is coming would be bad enough for me.

taper the done down to at least 40mg, switch to a short acting op for a 5-7 day stretch then switch to the sub. take your last dose of the short acting opiate at around 4pm the last day, take a benzo to help get you to sleep and hopefully stay asleep as long as you can, then the next morning around 8-10AM will be 16-18 hours. depending on how long you can sleep in, you may only have to endure around an hour or 2 of WD, then make the switch. what i would do anyway (if i was in this position) the friend i mentioned above used subutex and did a 13 hour switch from oxycodone and it worked quite well. no precipitated WD and after initial induction and finding the right dose which took around 3 days to get stabilized, he was fine on around 8-12 mgs of sub per day. this is coming from around 300mgs of oxycodone per day for a legitimate condition. he is still in pain, but is not taking nearly the amount of oxy he was in the past. he went from daily use, to the sub taper, and now only takes oxycodone on days that are really bad pain wise when he can not move. seems to have worked well for him in my opinion. he is now at the point that he does not have to worry about WD since he is no longer taking opiates daily, but does have to deal with constant pain still.

WAIT UNTIL IN W/D
only then dose 2mg sub
sublingually
wait 20minutes
dose 2mg increments
until 'well'

you will not prob get any buzz
but the sick will go away
6-8mg will prob be enough
use a little as possible.
