+Hey I think this may actually be the first THREAD I have started...never realized that I mostly just respond to what is going on around here already...

bottom line...I need HELP ASAP... will tell you all later how I got screwed AGAIN by the ROPs...

I have a third washing (I know, pathetic!) going on with some pods, stems, seeds, basically anything I could find to throw in the pot...

however, I still have 1/2 to 3/4 of a multi-dose bottle of 20mg/1 or 2 ml of Nubain...
I have never finished it/used it since it put me into w/d two times previously ~ last time was late spring!

now that I'm at the end of my rope I recall an article SOMEWHERE out there on the internet about safe dosing/administration of this specific drug in an opiate dependent patient....along the lines of give via IV 1ml, wait for 20-30 min. then repeat, etc..

but sadly I can't hardly stand to search for it as I'm so miserable right now...anyone want to help a brother(sister, lol..) out??

If nubain is a mixed agonist/antagonist (which is just about the only reason it would precipitate withdrawls) then it will keep you out of w/d's if taken properly. The only mixed ag/antag's I know of are staldol and buprenorphine. But Nubain may very well be one, too.

Follow normal subutex dosing directions- basically, wait until you can't stand the w/d's anymore before dosing. Wait at least 18 hours from your last dose, longer if it's tea.

You are correct, nalbuphine is a mixed agonist/antagonist. It has agonist activity at the kappa and sigma receptors, and antagonist at the mu receptors, which means it would greatly antagonize effects of pod/seed tea. I agree with tm420, I would wait until you absolutely cant stand the w/d's before administration, because it will initiate w/d's.

As far as administaration, the usual adult dose is 10 mg's in a 70 kg patient. According to the PDR, you should titrate the dose at 3mg over a 10-15 min period, and not to exceed 20 mg's in a non-tolerant patient.

It also states that the respiratory depression is equal to the of an equalgesic dose of morphine, but is not as pronounced in higher doses than is seen in high dose morphine administration.

So be careful with it. I would start low, and slowly increase, because the higher the dose, the greater the chance that it will cause w/d's, as well as respiratory depression.

P.S. I already stated this, but I cant stress it enough, administer the dose very slowly, because if you start having adverse reactions, and are put into w/d, the less nalbuphine you have in your system, the less severe the w/d's will be, so just take it slow.
Sorry if I seem like I am preaching, but I have seen people who have used too much buprenorphine for this reason, and the antagonist action was horrible, so be safe. Peace
