SWIM has a pretty cool doc he goes too for his upper back pain. He feel down some stairs a while back and finally went to the doc, turned out hes got something wrong. Doc wrote a script for lorcet 10, but SWIM is curious is there is anyway to possibly move up the charts a little bit. Aside from the lorcet not completely keeping the pain away from SWIMS tolerance to them, SWIM is a little worried that the prolonged use of APAP will not be the best thing in the long run. Any suggestions?

First time seeing the physician regarding this particular problem?

If he/she scripted you hydrocodone @ a dose of 10/650 (If I remember correctly you said Lorcet, right?), then it would be ill advised for you to push him for a higher dose, IMO. It's fairly uncommon for a doctor to prescribe that dose at the first visit, at least around here that's abnormal....but then again, WTF do I know - most docs in PDX are scared shitless to script narcotics.

You should probably do what most of us (CP patients) have done, and work your way up the 'script ladder' without pushing for dose increases. It looks bad and may get you cut off completely, depending on your diagnosis...you didn't share the exact injury/problem's diagnosis/prognosis, so it's hard to say. I'll advise you to take your time.

Let 3-4 months of scripts roll out until asking for an increase or med change....I waited almost a year before I actually asked for an INCREASE, and the doctor had no problem with it.

You have a pretty good thing going, and I'd hate to see you post back that you've been reduced to Ibuprofen 800's ;)

Most importantly:

-What's your tolerance like: How many mg's at a time gets you going?

-How long is 'prolonged'?

-Have you considered/been informed of cold water extraction (CWA) techniques?

A lot of people on here swear by them (CWA's)...personally,I don't do it because I can't afford to waste good drugs, so I just take the fucking pills, you know?

In the end, it's your liver. I've routinely administered myself with up to 9500 mgs of acetaminophen for 8-10 day stretches and had zero problems, but people are different, and once again: your liver is your responsibility.

Better to err on the side of caution then to be a liver transplant candidate, my friend...because that's how it happens - usually by the time you show symptoms, it too fucking late.

if you've had pain for over 6 months (chronic) and a record of scripts i would see a pain doctor...

i forgot to add- talk with your doctor and tell him you want to see a specialist (aka pain doctor)...he'll give you a referal if he's a good doc

Ok, so heres the story. I have had back problems for years, and about 4 months ago, I fell down the stairs and did something seriously wrong. I figured the pain would go away, but after two months, things just kept getting worse. I went to my doc about it and he was super cool. I have been going to him for the last two years for my ADD/Anxiety problems, but since the back incident, I have quit taking my adderall and I havn't taken my xanax in over 7 months or so. I have a lot of pain problems, from wrists to knees, to my back, and about everywhere else on my body. But my back is what is currently the majority of my problems. I have acquired all these injuries in my short 22 year old life, from the lovely sport of motocross. I have a fairly high tolerance from all the oxycodone that I have had over the last 3 years from a shoulder surgery and a horrible wrist break. I really need about 20mg's of oxy to get me going and to actually get some relief. And that is the absolute lowest. I prefer around 40. That puts me in a great mood and gives me near complete relief. I'm the same way with the CWE, I have a hard time just not going ahead and taking the things. They hydrocodone just isn't doing what I need it to. SWIM is currently scripted 90 Lorcet 10 for severe pain, 90 Tramadol 50's for moderate, 90 Parafon Forte for muscle spasms, and 90 Naproxon for mild pain and inflamation. Nothing works at all but opiates, partly because of SWIMS tolerance and partly because of the severe pain. I will be going in for and MRI this week to look inside there and see what the deal is. This will be the 3rd month of scripts.

The thing is, I do not have time for surgery so even if there is damage, it can't be repaired immeadiatly. The pain is in my upper back. I have thought about going to a pain management doctor, but I'm afraid when they see a 22 year old person walk in, the thoughts start flying through their heads, when in all reality, I am seriously in pain.

Please help.

Please help.People already did. If your GP refers you to a PM doc and sends over all of your records, I don't think you have to worry about "thoughts flying through their heads."

If you've been prescribed oxycodone off and on for the past three years, I wouldn't hesitate to bring it up to the PM doc (providing you have the proper documentation showing that you were actually prescribed it).

I wasn't scripted oxycodone for my back pain, it was immeadiatly after a broken wrist and a shoulder surgery.

the thing about waiting that long for med increases might be valid with some doctors, but if the doc has no idea what your tolerance is, as well as how much pain youre in, there shouldnt be anything wrong with saying 'it helps a little, buuuut....'

i told my prn off the bat i used to to be a heavy opiate user, was looking for bupe for pain, went home with fent patch, and increased at least every other week since then. shes not a croaker, just understanding

i wouldnt push it too far in a new situation like yours, but don't hesitate to say something if it's not covering your pain, especially for months
