At another ER last night, I was talking to the nurse who told me that 1mg of hydromorphone IV/IM is equal to 4mg hydromorphone PO...

I was given 2mg IM and I felt okay, but still had some damn pain... knocked it down a few points though.

that conversion seems about right from what I recall

1 mg iv/im/sc converts to 5 - 6 mg po (depending on how generous of a conversion you use)

Seems about right to me. It's been quite some time since I've had hydromorphone though.

Good to hear, Thanks all... I'm seeing a primary tomorrow that my girl got me into... I've also got my PM records here (finally) but they're missing my last dose upgrade to 15mg..so I gotta call prior..
